Clinical Coordinator III (RN) - PACC (HHT)

Job Description

Position SummaryThe Clinical Coordinator III (RN) will provide patient-centered nursing care directly, in addition to triaging patients remotely and helping coordinate testing and visits to providers across numerous specialties. The Nurse Coordinator will assist with clinical research study enrollment, participant monitoring, coordination and database management. They will work closely with the medical directors of the HHT program, as well as with other providers across a multidisciplinary team. ResponsibilitiesServe as the coordinator of a multidisciplinary team.Provide support to patients, assist physicians with procedures, and educate families.Maintain a safe and therapeutic patient care environment.Work closely with physicians to assure high-quality care of patients in the practice, and those under cross-coverage responsibility.Ensure relevant Telemed and front desk messages are addressed by the end of each day.Check diagnostic testing results for patients and take the appropriate measures to manage abnormal findings.Communicate test results with patients.Provide technical assistance to other associates seeking input for authorization of tests associated with the care of patients in the practice.Record all patient interactions in Epic.Develop and participate in the education of patients and families on HHT management.

Education

Bachelor's degree or equivalent in education and experience, plus four years of nursing experience
